I don't think Hulk is actually lifting the hammer at all, he's hitting Thor with his own hand which is gripping the hammer, in much the same way that older siblings have done for decades.

did you not see the blood on the hammer? besides if he didn't lift the hammer, how's he supposed to be hitting Thor at all?
